Register As A New Patient

If you live within our Practice boundary we would be delighted to register you as a patient with us.  If you are unsure or have any further questions. please contact the practice or e-mail admin.w97023@wales.nhs.uk

How to Register

To register as a patient at Woodlands Medical Centre you need to complete a GMS1W GP Registration Form. These can be picked up from reception along with a medical history questionnaire.  Once completed, drop the forms back to us and we will register you as a patient with us. 

If you have a repeat prescription from your previous surgery, please attach this to your completed forms so that we can add the medication to our records. 

Temporary Patient Registrations

If you fall ill while away from home, you can receive emergency treatment from a local GP Practice for 14 days.  After 14 days, you will need to register as a temporary or permanent patient.  

Once registered as a temporary patient you will remain a patient of you permanent GP and this registration will last for 3 months. 

You cannot register as a temporary patient at a practice in the area where you are already registered.
